What is the Bundled Care Program?

The Bundled Care Program is a provincial initiative in Ontario designed to provide patients who are undergoing total hip, knee, or shoulder replacement surgeries with seamless, integrated post-surgical care. Under this program, hospitals partner with physiotherapy clinics to ensure you receive physiotherapy sessions following your surgery. These sessions focus on education, exercises, and progressing you towards an independent home exercise program.

  • Sessions are exercise-based with the goal of improving mobility, strength, and function
  • Direct billing to your hospital

Who is the Bundled Care Program for?

  • The Bundled Care Program is for Ontario residents only.
  • Patients who are undergoing a first-time hip, knee, or shoulder joint replacement surgery.
  • Patients who have been referred by a hospital post-surgery for physiotherapy.
  • Patients looking for integrated post-operative rehab through their hospital's partnership with local physiotherapy clinics.

What to Expect During Your First Visit

  • Your first physiotherapy visit should happen within 7–10 days of your surgery.
  • Please bring:
    • The hospital referral or paperwork
    • The prescription from your surgeon
    • Your Ontario Health Card

During your first visit, we will:

  • Complete an initial assessment.
  • Begin customized rehabilitation exercises to get you back on track.

What if I Need More Sessions?

We aim to support your recovery under the Bundle of Care Program. If your treatment needs extend beyond the covered sessions, additional physiotherapy can be arranged through private payment or your extended health benefits. We’ll work with you to create a personalized treatment plan aligned with your recovery goals.
